Job summary

NHS GP Practice is seeking a Care Navigator to be the first point of contact for patients, helping them access the right service. You will triage requests, support appointment booking, and signpost to NHS and community services, coordinating with clinicians and administrative staff.

You will maintain accurate patient records in clinical systems, ensure GDPR compliance, and help manage patient flow and engagement to improve access to GP services.

Qualifications

• Strong communication and customer service skills.
• Good IT skills (clinical systems desirable).
• Ability to prioritise workload in a busy environment.
• Understanding of NHS pathways desirable.
• Experience in GP practice or healthcare setting desirable.

Responsibilities

• Act as first point of contact for patients (phone, face-to-face, online).
• Triage patient requests and direct to appropriate clinician or service.
• Support appointment booking and manage patient queries.
• Signpost to NHS and community services.
• Maintain accurate patient records using clinical systems (EMIS/SystmOne).
• Ensure GDPR and confidentiality compliance.
