TheHomewell Practice is expanding its Care Coordinator-Navigator team in Havant. The role is part-time (16 hours per week) with 2:30pm–6:30pm shifts on Monday, Wednesday, Thursday and Friday, delivering patient-focused care and ensuring timely access to services.
You will work in a busy GP practice, supporting communication across primary and secondary care, and will receive full training to develop in the role within a supportive team environment.

We are looking to recruit a Care Coordinator-Navigator towork 16 hours per week, joining our supportive andfriendly practice family.
You MUST be able to work from 2.30pm until 6.30pm on Monday, Wednesday, Thursday and Friday.

Experience in a busy customer facing role is essential, andexperience within a healthcare setting is desirable. This is a varied and fast pacedrole where no two days are the same. It is not a role that can be learnedovernight, so we are seeking someone who is motivated by the challenge andeager to grow. Full training will be provided.
The post holder will support the smooth running of thepractice by coordinating patient care, navigating individuals to the mostappropriate services, and ensuring effective communication across thehealthcare system.
You will present a positive, friendly, and professionalimage to patients and visitors, whether in person or over the phone. You willhelp patients access the right clinician or service in a courteous, efficient,and effective manner, while supporting communication between patients, theprimary care team, secondary care, and other healthcare agencies.
Handling incoming callsfrom patients and healthcare agencies
Supporting patients toaccess the right service, clinician, or pathway
Managing patientadministration, including registrations, deductions, and demographicupdates
Processing clinical andadministrative information from secondary care and other providers
Completingadministrative tasks as directed by clinicians and the leadership team
Welcoming and checkingin patients and visitors in a safe, polite, and efficient manner
Helping coordinate careplans, follow up actions, and referrals
Supporting proactivecare initiatives and population health management activities

Effectivesignposting to local services
Provide timelyfeedback to colleagues around issues relating to access to services
Applyskills and techniques (verbal and non-verbal) to resolve conflict, reducedistress and anger
Sharerelevant information, decisions and discussions made by health and social careteams, with the patient (and carer if appropriate)
Showinitiative to re-refer a person if known to services when appropriate contacthas not occurred with agreement and supervision of multidisciplinary team leadclinician
