Overview

Care Navigator role at TieTalent. We are seeking an experienced Care Navigator to provide support across GP Practices and Primary Care Hubs in Sefton and the wider Liverpool area. As a Care Navigator with PRIMARY CARE experience, you will be a vital link between patients, doctors, and other healthcare professionals.

Job Summary: We are looking for a skilled and dedicated Care Navigator to join our team on a contract basis. The Care Navigator will provide administrative support to GP Practices and Primary Care Hubs, ensuring that patients receive the best possible care. If you have previous experience as a Receptionist or Care Navigator within a GP or health centre, we would love to hear from you.

Responsibilities

• Provide administrative support to GP Practices and Primary Care Hubs
• Handle patient queries and provide information on services and procedures
• Work closely with healthcare professionals to ensure seamless patient care
• Maintain accurate records and databases using EMIS
• Offer patient navigation duties and provide support to patients

Qualifications

• Experience as a Care Navigator or Receptionist within a GP or health centre is preferred
• Strong communication and organizational skills
• Familiarity with GP administrative processes and EMIS or similar systems
