Job summary

NHS GP Practice is seeking a Care Navigator to be the first point of contact for patients, helping them access the right service. You will triage requests, support appointment booking, and signpost to NHS and community services, coordinating with clinicians and administrative staff.

You will maintain accurate patient records in clinical systems, ensure GDPR compliance, and help manage patient flow and engagement to improve access to GP services.

Qualifications
