Job summary

Doncaster Council is seeking a dedicated Supported Accommodation Pathway Coordinator (CDC) based at Civic Building to coordinate access to supported accommodation, assess individuals, and match them to suitable services. Applicants should have relevant experience in housing, homelessness, social care or related services, with skills in assessments, support planning, risk management, safeguarding, and multi-agency working.

This is a full-time, temporary role with a fixed end date.

Qualifications

• Experience in housing, homelessness, social care or related services essential.
• Experience of completing assessments, support planning, risk management, safeguarding, GDPR, and multi-agency working.
• Strong person-centered, strengths-based approach and ability to manage multiple referrals and partnerships.

Responsibilities

• Coordinate access to supported accommodation and ensure individuals are assessed and matched to suitable services.
• Maintain accurate records, share information, and produce reports for multi-agency teams.
• Manage referrals, placements and stakeholder relationships within tight timescales.
