Job summary

Heart of Kent Hospice is seeking a Social Worker for Specialist Palliative Care on a 6-month fixed term contract. The role is part-time (30 hours per week) within the Family Support Team, based at the Hospice or in the community.

You will assess adults and families facing life-limiting illness or bereavement, deliver holistic support, and work with a multidisciplinary team. Requirements include a Degree in Social Work and HCPC registration; a driving licence is essential.

Qualifications

• Experience with adults and families facing major life changes, trauma and loss.
• Must hold a Degree in Social Work or equivalent.
• Current HCPC registration is required.
• Driving licence and access to transport are required.

Responsibilities

• Develop group activities, support carers, and provide therapeutic support to adult patients and families.
• Undertake holistic assessments and prioritise caseloads.
• Collaborate with the multi-disciplinary team, including nurses, social care, welfare and spiritual lead.
