Job summary

Dorothy House Hospice is seeking a Rehabilitation Practitioner to join the Allied Health Professional team. The role is a part-time position (22.5 hours over three days) focusing on assessing needs, designing personalised exercise programmes and helping patients stay active in what matters to them.

You will work with physiotherapists, occupational therapists and clinical support staff to deliver rehabilitative palliative care, support students and volunteers, and contribute to audits and service

Qualifications

• BSc in a health or exercise-related subject such as Sport Science or Exercise Physiology.
• Evidence of continuing professional development in exercise or rehabilitation.
• Experience working with people with physical health conditions in healthcare settings.

Responsibilities

• Assess patients and develop personalised rehabilitation and exercise programmes.
• Deliver evidence-based exercise interventions to support independence, wellbeing and quality of life.
• Manage referrals and support delivery of rehabilitative palliative care services.
• Work closely with physiotherapists, occupational therapists and clinical support staff to provide coordinated care.
• Support patients, families and carers with practical and emotional support within the multidisciplinary team.
• Facilitate education, self-management strategies and exercise programmes for patients and carers.
• Support students and volunteers and contribute to their development.
• Contribute to audit, quality improvement, service development and research-informed practice.
• Maintain accurate records and ensure high standards of professional practice and governance.
