Dorothy House Hospice Care is seeking an Exercise Rehabilitation Practitioner to join our Allied Health Professional Team in Winsley, Wiltshire. You will design personalised rehabilitation and exercise programmes for patients with complex health needs, coordinating with physiotherapists and occupational therapists to support independence and quality of life.

Qualifikationen

• Excellent communication and interpersonal skills are essential.
• Strong organisational skills and the ability to prioritise workloads.
• Ability to work independently in a multidisciplinary setting.
• Demonstrable understanding of rehabilitative palliative care.

Aufgaben

• Assessing patients and developing personalised rehabilitation and exercise programmes.
• Delivering evidence-based exercise interventions to support independence and quality of life.
• Working closely with Physiotherapists and Occupational Therapists to provide coordinated care.
• Supporting patients, families and carers through practical and emotional support.
• Managing referrals and ensuring delivery of rehabilitative services.
• Contributing to audit, quality improvement and research-informed practice.
