Job summary

Dorothy House Hospice in Wiltshire is seeking an Exercise Rehabilitation Practitioner (Band 5) to join our specialist AHP team. This part‑time post works 22.5 hours across 3 days and is pro rata for the annual salary £32,073–£39,043.

You will assess patients, develop personalised rehabilitation programmes and deliver evidence‑based exercise interventions to support independence and quality of life within a multidisciplinary team. Interviews in early November.

Qualifications

• Experience delivering rehabilitative exercise for palliative care patients.
• Ability to assess patients and develop personalised rehab plans.
• Willingness to support students and volunteers.

Responsibilities

• Assessing patients and developing personalised rehabilitation and exercise programmes.
• Delivering evidence-based exercise interventions to support independence, wellbeing and quality of life.
• Working closely with Physiotherapists, Occupational Therapists and Clinical Support Assistants to provide coordinated care.
• Supporting patients, families and carers through practical and emotional support as part of the multidisciplinary team.
• Managing referrals and supporting the effective delivery of rehabilitative palliative care services.
• Facilitating education, self-management strategies and exercise programmes for patients and carers.
• Supporting students and volunteers and contributing to their development where appropriate.
• Contributing to audit, quality improvement, service development and research-informed practice.
• Maintaining accurate records and ensuring high standards of professional practice and governance.

WHAT WILL THE ROLE INVOLVE?
