Job summary

St Peter & St James Hospice in East Sussex seeks a Rehabilitation Support Worker to deliver rehabilitative palliative care under physiotherapists and occupational therapists. You will help patients reach personal goals in mobility, daily living and breathlessness, including home visits and group sessions at the Living Well Centre.

You will work within a multidisciplinary team, support carers, maintain records and coordinate workload.

Qualifications

• NVQ level 3 in Health and Social Care, or hospice/therapy/care experience.
• Experience in a care or rehabilitation environment is desirable.

Responsibilities

• Support rehabilitation programmes under therapists.
• Help patients reach rehabilitation goals to maximise independence.
• Deliver mobility, daily living, breathlessness management and transfer practice.
• Assist patients and carers with equipment use and home safety.
• Plan and deliver group activities and Living Well Centre programmes.
• Undertake home visits and risk assessments.
• Provide practical and emotional support to patients and families.
• Record progress and report changes to the supervising team.
• Manage workload, appointments and equipment orders.
• Collaborate with colleagues, volunteers and other health professionals.
• Participate in meetings, supervision and training.
