Job summary

Wyre Forest Health Partnership is seeking a motivated Practice Nurse to work 23 hours per week. The role delivers nursing assessments and high-quality care in line with practice policies, collaborating with GPs and the wider team.

Chronic disease management and mentorship of Health Care Assistants are key parts of the role, with a focus on improving patient care standards and service delivery.

Qualifications

• Registered nurse with current UK NMC registration.
• Experience in primary care or district nursing is desirable.
• Minimum 2 years post-registration experience.
• Experience in managing long-term conditions and chronic disease.
• Experience implementing and using clinical protocols and governance.

Responsibilities

• Undertake nursing assessments and provide care/treatment with GPs according to practice policy and protocols.
• Lead wound care, leg ulcers, dopplers and compression bandaging.
• Administer adult and paediatric injections under prescribed directives; manage vaccine storage and disposal.
• Provide immunisation services following appropriate guidance.
• Apply infection control measures when handling specimens and disposing of waste; manage sharps injuries.
• Support and mentor Health Care Assistants and contribute to quality improvement in patient care.
