Job summary

Wyre Forest Health Partnership is seeking a motivated Practice Nurse to work 23 hours per week at Church Street Surgery, delivering nursing assessments and high-quality care in line with policies, collaborating with GPs and the wider team. The role focuses on Chronic Disease Management, mentoring Health Care Assistants, and improving patient care in our local population.

Qualifications

• Registered first level nurse required.
• Clinical supervision training and experience.
• Knowledge of needs of patients with long-term conditions.
• Awareness of clinical governance issues in primary care.
• Basic IT skills.

Responsibilities

• Undertake a range of nursing assessments with GPs; provide care/treatment.
• Support chronic disease management and mentor Health Care Assistants.
• Maintain infection control and safe handling of specimens.
