Job summary

Eightlands Surgery in Dewsbury is seeking a motivated Practice Nurse with recent Primary Care experience. If you are looking for a forward-thinking environment, this role offers a supportive team and opportunities to develop in chronic disease care.

You will perform long-term condition management, health checks, cervical screening, vaccinations, ECGs and phlebotomy, with flexible part-time hours (30 per week) and NHS pension support.

Qualifications

• Must be a registered Nurse with full NMC registration.
• Post graduate diploma or degree (Chronic Disease Management).
• Experience of working in a primary care environment.
• Experience of chronic disease management.
• Experience of working as a practice nurse or community nurse.
• Experience of managing elderly and housebound patients.

Responsibilities

• Implement processes for the effective management of patients with long-term conditions using evidence-based practice.
• Work as a senior member of the practice multidisciplinary team to ensure nursing services are delivered to the entitled patient population.
• Contribute to clinical areas such as infection prevention and control, health promotion, chronic disease management.
• Support the practice management team in reviewing and delivering clinical policy and procedure.
