Job summary

Birmingham Community Healthcare NHS foundation Trust is seeking a qualified practice nurse to join our team across three partner sites in Birmingham. The role focuses on chronic disease management, immunisations, travel health, and wound care, with a strong emphasis on patient-centred care.

The successful candidate will be a registered nurse with current NMC registration and experience in General Practice, able to work independently and as part of a multidisciplinary team.

Responsibilities

• Chronic disease management, including long term condition reviews
• Cervical cytology
• Childhood and adult immunisations
• Travel health advice and vaccinations (where appropriately trained)
• Wound care, dressings and removal of sutures/clips
• ECGs and other clinical procedures
• Health promotion and patient education
• Supporting patients with preventative healthcare and lifestyle advice
• Maintaining accurate clinical records using the practice clinical system
• Contributing to infection prevention, clinical governance, and quality improvement initiatives

• Maintaining accurate clinical records using the practice clinical system


• Contributing to infection prevention, clinical governance, and quality improvement initiatives

The successful candidate will:

• Be a registered nurse with current NMC registration


• Have previous experience working in General Practice


• Be confident in delivering chronic disease management and routine practice nursing duties


• Have excellent communication and interpersonal skills


• Be organised, adaptable and able to work independently as well as part of a multidisciplinary team


• Demonstrate a commitment to delivering high-quality, patient centred care


• Be willing and able to work across all three practice sites within the partnership
