Job summary

Greenhead Family Doctors in Huddersfield is seeking a part-time practice nurse (7.5 hours per week, Friday preferred) to deliver comprehensive nursing care. You will implement and review care plans, manage chronic diseases, conduct immunisations including travel vaccines, and support infection control alongside the GP team.

The post requires autonomous working within a primary care setting, with RN registration and CPD commitment.

Qualifications

• Experience in working in a primary care environment.
• Experience of chronic disease management.
• Experience of working autonomously.

Responsibilities

• To follow agreed clinical protocols and refer to GPs as appropriate.
• Vaccination and health advice prior to travel.
• Routine and childhood immunisations.
• Vaccine management, reporting, monitoring and storage.
• Well person clinics and lifestyle advice.
• Chronic disease annual reviews (diabetes, COPD, asthma, hypertension).
• Cervical cytology duties.
• Lead on infection control and ensure guideline compliance.
