• Strong communication, relationship-building, and partnership working.
• Organised and self-motivated with caseload management experience and safeguarding/risk practices.

Responsibilities

• Manage a caseload of adults with complex needs.
• Provide person-centred, trauma-informed support to enable tenancy sustainment and independent living.
• Carry out support and risk assessments and create tailored support plans.
• Work with partner agencies to access services and safeguard vulnerable adults.
