Job summary

Cornerstone Community Care in Perth is seeking a Lead Practitioner on a part-time basis (20 hours per week) to lead and mentor teams delivering high‑quality care and support to service users.

You will manage rosters, provide guidance, and help ensure services run within budgets and contractual requirements, while upholding professional standards and person‑centred care across our Perth services. This role may involve travel to Kinross and on-call duties.

Qualifications

• Experience of managing people.
• Experience of rota management.
• Knowledge of delivering high-quality, person-centred care in Health & Social Care.
• Familiarity with SSSC Codes of Practice and Health & Social Care Standards.
• Ability to lead others and work collaboratively.
• Strong communication skills, both verbal and written.

Responsibilities

• Lead and inspire teams of Support Practitioners.
• Provide guidance and mentorship to ensure professional, accountable care delivery.
• Contribute to budget and contractual compliance with Service Lead.
• Support service operations across Perth and Kinross sites.
