Job summary

Cornerstone Community Care in North Lanarkshire is seeking a Lead Practitioner to lead and mentor a team of Support Practitioners in our transitional service for young people, helping them develop confidence and life skills while aiming for independence.

The role is part-time (25 hours), fixed-term for 12 months to provide maternity cover. You will work closely with the Service Lead to ensure high-quality, person-centred support and adherence to professional standards.

Qualifications

Experience of managing and supporting people in health and social care settings.
Understanding of delivering person-centred support to young people.
Knowledge of SSSC Codes of Practice and Health & Social Care Standards.
Commitment to professional boundaries and accountability.

Responsibilities

Lead, support and inspire a team of Support Practitioners to deliver high-quality, person-centred support.
Provide guidance and mentorship to the team to develop confidence, independence and life skills in young people.
Work with the Service Lead to ensure the service operates effectively and within requirements.
Ensure compliance with safeguarding and professional standards.
