Job summary

Nottinghamshire Healthcare NHS Foundation Trust is seeking a Band 6 Care Coordinator for the South Early Intervention in Psychosis Service in Nottingham. The post is fixed-term for 12 months (maternity cover) and part-time (22.5 hours per week, 9-5, Mon-Fri).

The role will involve community-based work with young people aged 14-65 experiencing FEP or ARMS, based at The Stonebridge Centre. The post requires a degree-level professional qualification in Nursing, Occupational Therapy, or Social Work,

Qualifications

• Core professional qualification to degree/diploma level in Nursing or Occupational Therapy.
• Evidence of continued professional development.
• Nurse registrants: completion of Preceptorship.

Responsibilities

• Support and manage a caseload of service users experiencing first episode psychosis
• Assess and screen service users referred to the EIP Team
• Collaborate with the MDT to determine appropriate management plans with service users and families
• Use standard outcome measures to monitor recovery (HoNOS, QPR, DIALOG)
• Travel across Nottinghamshire to support service users, families and carers
