Job summary

Service Care is seeking a Band 6 Community EIT Clinical Worker to join the Early Intervention Team in Derby. The role involves managing a caseload, conducting CPA assessments, and delivering community-based care with occasional travel.

A driving license is essential for travel between Derby City and South Derbyshire. This 12-week contract offers a competitive rate of £27 per hour LTD, with potential extension based on performance.

Qualifications

• Must hold a valid nursing qualification as a Community Psychiatric Nurse.
• Must be able to travel to community sites across Derby City and South Derbyshire.

Responsibilities

• Manage a caseload of approximately 15–20 patients, delivering community-based care and treatment.
• Conduct CPA assessments, care planning, and documentation.
• Support initial assessments as a second clinician, adhering to the EIT two-week waiting standard.
• Deliver assertive outreach across Derby City and South Derbyshire to engage and support service users.
• Work with the wider clinical team to ensure safe, effective, and timely patient care.
• Complete community visits with travel as required; driving license essential.
• Maintain detailed records to ensure compliance with clinical standards.
