ONSIDE is seeking a Community Mental Health Link Worker in Redditch to support people with severe mental ill health. The role involves non-clinical, person-centred support, navigating health and social services, and connecting individuals with opportunities to build confidence and wellbeing.

The post holder will work with Mental Health Neighbourhood Teams, manage a caseload, and provide guidance to volunteers. Driving and access to a vehicle are essential.

Qualifications

• Experience supporting people with severe and enduring mental ill health.
• Ability to provide person-centred one-to-one and group support.
• Understanding safeguarding, confidentiality and data protection.
• Knowledge of health and social care pathways and community services.

Responsabilités

• Provide person-centred one-to-one support and access to group or peer support.
• Help individuals address wider issues affecting wellbeing (housing, debt, diet, activity).
• Signpost, refer and navigate voluntary and community support.
• Maintain case records and reports using Rio and CharityLog.
• Collaborate with teams and VCSE partners to provide joined-up support.
