Job summary

Derbyshire Community Health Services NHS Foundation Trust is inviting applications for a Community Nurse Band 5 based in Long Eaton to join our community nursing teams. As a Registered Nurse, you will assess, plan, implement and evaluate personalised patient care in patients' homes, coordinating with general practice, rapid response and community therapy colleagues.

You will promote self-care, maintain privacy and dignity, and provide guidance to junior staff while supporting Senior Community

We are looking to recruit full or part time registered nurses to work within the Community Nursing Teams based in Long Eaton in South Erewash
