CCH Group is seeking a Care Coordinator in the Darwen area to help organise and deliver high-quality home care. The role includes managing referrals, creating rotas for care workers and coordinating with social workers and GPs to support service users.
Salary is £15,218 plus on-call payments, with Monday to Friday hours from 9am to 1:30pm and a rotating on-call schedule. Driving licence is required and training and career development opportunities are available.

Experience & Organisation: You are an efficient, organized Care Coordinator passionate about community care. You excel in managing rotas to impeccable standards and pride yourself on building strong relationships with care workers.
