Job summary

Swan Medical Group is seeking an experienced Acute Care Nurse Practitioner to join our team across all sites. The post focuses on acute minor illness care, with telephone triage and in-person assessments, aiming to prevent unnecessary A&E visits and hospital admissions.

Qualifications

• Experience in acute minor illness care in a clinical setting.
• Strong communication and IT skills to manage patient interactions and record-keeping.
• Ability to work across multiple sites and manage time effectively.

Responsibilities

• Conduct face-to-face assessments and treatment for minor illnesses, including telephone triage.
• Work across Swan Medical Group sites to provide consistent patient care.
• Support commissioning of safe, high-quality patient care and triage decisions.
