Job summary

Swan Medical Group is seeking an Acute Care Nurse Practitioner to join its established team across five Hampshire GP surgeries. The role focuses on face-to-face assessment and treatment of minor illness, plus phone triage, with the aim of keeping patients out of A&E and hospital.

Responsibilities

• Face-to-face clinical assessment and treatment of patients with minor illness.
• Telephone consultations and triage.
• Working across all sites.
• Contributing to acute minor illness care and the Home Visiting Service, aiming to prevent A&E visits and hospital admissions.
• Organising clinic and managing time so patients are seen promptly without compromising care or safety.
• Delivering care and treatment autonomously within the clinical setting.
