Job summary

Swan Medical Group seeks an acute care Nurse Practitioner to join a team of Nurse Practitioners and Paramedics across five GP sites. The role focuses on face-to-face and telephone assessment, triage and treatment of minor illness, plus the Home Visiting Service, aiming to keep patients out of A&E and hospital.

Applicants need NMC registration and A&E or urgent care experience; the post offers development and ongoing training within a supportive clinical team.

Qualifications

• NMC registration is required.
• Experience in A&E or urgent care.
• Strong organisational and time management skills.
• Excellent communication and a team-player approach.
• Triage and rapid assessment skills.
• Qualification modules in history taking and physical assessment.
• Non-medical independent prescriber qualification is a plus.

Responsibilities

The work centres on face-to-face and telephone assessment, triage and treatment of minor illness, plus the Home Visiting Service, aiming to keep patients out of A&E and hospital. Practitioners work autonomously, and the group trains doctors. Applicants need NMC registration and A&E or urgent care experience.
