Job summary

Jersey Hospice Care invites applications for a Bank Nurse Care Coordinator to support the Living Well bank team, working with Senior Nurse Care Coordinators and the Specialist Palliative Care Team across Community, Hospice and Hospital settings.

The role focuses on delivering person-centred care for patients with life-limiting conditions in the last year of life, with emphasis on holistic assessment and care planning.

Responsibilities

• Identify and support patients and families with life-limiting conditions in the last 12 months of life.
• Signpost and coordinate care, working closely with other agencies.
• Undertake holistic needs assessments and plan and implement programmes of care aligned to present and future wishes and advance care planning.
• Work autonomously within a team of Senior Nurse Care Coordinators and with the Specialist Palliative Care Team.
