NHFT is seeking an experienced Specialist Palliative Care Clinical Nurse Specialist to provide expert advice and clinical support through the SPOA and Urgent Community Response service in Northampton. You will work with patients, families, GP practices and care partners to manage complex end-of-life needs at home, aiming to prevent unnecessary hospital admissions.

The role is part-time (22.5 hours per week), shifts 12pm-8pm, with a pro rata salary of £49,387 to £56,515 per year.

Qualificações

• NMC registered nurse.
• Evidence of specialist palliative care qualification or significant experience.
• Evidence of ongoing CPD.

Responsabilidades

• Provide specialist palliative care advice via SPOA and Urgent Community Response.
• Support patients and families to stay at home and prevent hospital admissions.
• Collaborate with GPs, care homes and wider system partners.
• Contribute to education, service development and palliative care capability.
