Job summary

Carers’ Resource in Bradford invites applications for a Specialist Carer Navigator (Palliative Care & Discharge Planning) based at Bradford Royal Infirmary and our Shipley office. Qualifications

• Experience supporting carers of people receiving palliative or end-of-life care is desirable.
• Ability to explain discharge options and navigate health and social care systems.

Responsibilities

• Provide specialist support to carers of people receiving palliative or end-of-life care within the hospital setting.
• Offer information and guidance to support carers in decisions around discharge from hospital.
• Support carers to understand palliative care services, advance care planning, hospice provision, community nursing support, and available resources.
• Provide emotional support to carers facing difficult decisions about deterioration and end-of-life care.
• Inform carers of their rights and available bereavement services where appropriate.
• Collaborate with hospital wards, discharge teams, palliative care teams, social workers and community services to support safe discharge planning.
• Assist carers in identifying and evaluating discharge options, including returning home with support, care homes or hospice care.
• Facilitate discussions between carers and professionals to support discharge planning.
• Identify barriers to discharge and work with agencies to find practical solutions.
