Job summary

Frimley Health NHS Foundation Trust is seeking a Band 6 Children’s Community Nurse to join the Community Nursing team. This full-time, permanent role focuses on delivering high-quality, compassionate paediatric care in the community and supporting families to manage their child’s health needs outside hospital settings.

Essential criteria include registered Sick Children’s Nurse with mentorship, current paediatric experience, and strong communication across MDTs.

Qualifications

• Current paediatric experience.
• Ability to work across sites and MDT.
• Commitment to safeguarding and audit where applicable.

Responsibilities

• Take responsibility for nursing management of a caseload of children in the community.
• Be the key health professional and care co-ordinator for children and young people at home.
• Provide high-quality patient care to children and families in the community.
• Train families to manage medical conditions at home.
• Support care planning, delivery and evaluation to meet standards.
• Facilitate early discharge and aftercare in the community.
• Respect families' wishes regarding end-of-life arrangements.
