Job summary

Epsom and St Helier University Hospitals NHS Trust and Chelsea and Westminster Hospital NHS Foundation Trust are looking for a PAC MDT service coordinator. The role requires a full, clean driving licence and access to a car for home visits. You will work to coordinate referrals, arrange GP/complex meetings, and keep patient records up to date.

Promote the PAC MDT service at meetings, adhere to service policies, and guide patients to suitable services to support their empowerment and progress.

Responsibilities

• Participate in weekly referral triage meetings to assist in identifying appropriate referrals.
• Coordinate GP/complex meetings to ensure they are arranged, coordinated, and communicated with timely information.
• Update patient records and universal care plan to ensure coordinators and other service providers have access to updates/progress.
• Present yourself as an ambassador for the service and attend external practice/service meetings to promote the PAC MDT service when necessary.
• Attend PAC service meetings as required.
• Ensure PAC service policies are fully adhered to.
• Direct patients to suitable services and resources to support their empowerment and enable them to manage their own health.
