Responsibilities

• Participate in street outreach to engage with young people who are rough sleeping or at risk of rough sleeping.
• Provide effective triage, assessment and ongoing support based on individual needs.
• Support young people to remain in their existing accommodation or secure suitable alternative housing.
• Work closely with statutory and voluntary agencies to ensure young people's needs are met.
• Support young people experiencing complex needs, including trauma, poor mental health, offending and substance misuse.
• Help young people develop positive social networks, resilience and independent living skills.
• Support young people to move away from the streets and towards stable accommodation and recovery.
• Maintain accurate records and contribute to positive outcome-focused support planning.

• B9 4DY Location: B9 4DY Pay: £13.50 per hour + Holiday Pay Hours: Monday-Friday, 9:00am-5:00pm Contract: Temporary to Permanent About the Role We are currently recruiting for an Outreach Navigation Worker to support homeless young people and care leavers who are rough sleeping or at risk of homelessness.

Driving: Full UK driving licence and access to a vehicle is desirable but not essential.
