Job summary

BRC Jobs is seeking an Outreach Support Worker in Monmouthshire to help homeless and socially excluded individuals gain access to housing, health and welfare services. You will engage with people in community settings, build trust and provide tailored advice, working closely with local agencies to ensure wraparound support.

Qualifications

Outreach Support Worker

Location: Monmouthshire
Position Type: Full Time (Mon-Fri)

About the Role

As a Outreach Worker, you will be at the forefront of supporting individuals experiencing homelessness, rough sleeping, or social exclusion. You’ll engage directly with people in the community, often in non-traditional settings, to offer support, build trust, and connect them with vital services. Your role will focus on:

  • Actively engaging with people who are vulnerable, isolated, or hard to reach.
  • Providing tailored advice and support around housing, health, benefits, and wellbeing.
  • Encouraging individuals to access appropriate services and take steps toward independence.
  • Working in partnership with local agencies, charities, health providers, and housing services to ensure wraparound support.
  • Maintaining accurate records and contributing to a team approach in delivering high-quality support.
What We’re Looking For
  • Experience in outreach, support work, or working with vulnerable adults.
  • Strong communication and relationship-building skills, with a non-judgmental approach.
  • The resilience and empathy to support people facing complex challenges, including homelessness, mental health issues, substance misuse, and trauma.
  • A flexible, proactive attitude with the ability to work independently and as part of a team.
  • Knowledge of housing, safeguarding, and local support networks is desirable.
  • A full UK driving licence (and access to a car) is essential.
