Job summary

Harrogate and District NHS Foundation Trust is seeking highly motivated Advanced Clinical Practitioners to join the Out of Hours service on a bank basis. The role offers adhoc shifts across Harrogate, Ripon, Northallerton and Catterick, with operational hours from 6pm–8am on weekdays and 24hr cover from 6pm Friday to 8am Monday and bank holidays.

The successful candidate will perform telephone triage, clinical assessments and referrals using advanced skills to manage undifferentiated conditions,

Responsibilities

• Provide telephone triage and clinical assessment.
• See patients in the primary care centre and undertake home visits where clinically appropriate.
• Assess, diagnose and treat or refer patients presenting with undifferentiated and undiagnosed conditions.
