Responsibilities

• Lead day-to-day operational management of the Care Coordination Centre.
• Ensure referrals are logged, triaged, and allocated within agreed timeframes.
• Maintain data quality and ensure compliance with information governance.
• Oversee staff performance, supervision, and development.
• Coordinate multi-agency work and escalation procedures.
