Job summary

Norfolk and Suffolk Integrated Care Board (ICB) is seeking a Band 6 Registered Nurse (Mental Health, Adult or Learning Disability) to lead complex NHS Continuing Healthcare assessments and case management. You will apply clinical judgement and leadership to ensure fair, person-centred decision making for individuals with significant and complex health needs.

Qualifications

• Clear communicator with writing, report writing and presentation skills.
• Ability to influence and persuade across multidisciplinary teams.
• Experience of assessing people with complex needs and producing evidence-based reports.
• Knowledge of the NHS Continuing Healthcare and related frameworks.
• Ability to organise an autonomous caseload and work to tight deadlines.

Responsibilities

• Coordinate timely, comprehensive assessments for NHS CHC and funded nursing care.
• Prepare clear, evidence-based assessment reports and eligibility recommendations.
• Participate in MDT meetings and decision-making processes.
• Develop person-centred care and support plans for funded individuals.
• Support budgets and safe planning in line with national criteria.
