Job summary

Trident Group is seeking a Lead Worker / Support Worker in Birmingham to provide early, person-centred support for adults facing complex vulnerabilities. The role involves community-based work in homes, supported accommodation, and other settings across the city.

You will manage a caseload, perform needs and risk assessments, and work with partner agencies to coordinate access to services, with a focus on independence, recovery and reduced social isolation.

Qualifications

• Experience of supporting vulnerable adults with multiple and complex needs is desirable but not essential.
• Excellent communication, engagement and relationship-building skills are required.
• Ability to manage a diverse caseload, prioritise demands and work independently in community settings.
• Good understanding of safeguarding principles and relevant legislation relating to vulnerable adults.

Responsibilities

• Provide person-centred support to enhance recovery, independence and resilience in adults.
• Manage an active caseload and maintain up-to-date records and outcomes.
• Undertake needs and risk assessments in line with eligibility criteria.
• Deliver flexible support and adjust frequency/intensity as required.
• Enable client involvement in decisions and goal setting.
• Help clients access health, mental health, and substance misuse services as needed.
• Collaborate with housing, DWP, health and other services to coordinate support.
