Job summary

Barnet Council is seeking an enthusiastic Occupational Therapist to join the Transitions Programme, supporting young people aged 14 to 25 through the move from Children and Family Services to Adult Services. You will deliver assessments, interventions and care coordination to help them achieve independence and meaningful participation.

Are you a passionate Occupational Therapist who wants to make a real difference in the lives of young people as they move into adulthood?

We are seeking an enthusiastic and motivated Occupational Therapist to be part of our Transitions Programme. This role offers a rare opportunity to influence life‑changing outcomes. You will work alongside young people and families at a pivotal stage of life, supporting the journey towards greater independence, employment opportunities, community inclusion, and successful adult living. You will work with young people aged 14 to 25 who have a range of disabilities including learning disabilities, autism, physical disabilities, neurodevelopmental conditions, or complex health needs.

Working with a range of professionals across Adult Service, Children and Family Service, Health, and Education, you will support young people and their families through the transition from Children's and Family Services to Adult Services, ensuring they are empowered to achieve greater independence, choice, and meaningful participation in everyday life.

About the Role

Unlike many occupational therapy roles, this position spans both children's and adult pathways, allowing you to build long-term relationships with young people and influence transition planning from age 14 through to adulthood. You will provide occupational therapy assessments, interventions, and care coordination, helping individuals develop the skills and confidence needed for adulthood.

Working collaboratively with social care, health, education, housing, and voluntary sector partners, you will play a vital role in promoting positive outcomes in areas such as:

• Independent living
• Daily living skills
• Behavioural interventions
• Education, training, and employment
• Community access and participation
• Housing and accommodation
• Assistive technology and adaptations
• Sensory needs and wellbeing

You will contribute to transition planning from age 14 onwards, ensuring assessments and interventions are person‑centred, strengths‑based, and focused on achieving meaningful outcomes.

You will be supported by an experienced Lead Occupational Therapist and benefit from regular supervision, reflective practice, and professional development opportunities.

We are looking for a qualified Occupational Therapist who is committed to empowering young people and delivering excellent outcomes and is looking for a genuine opportunity to influence practice, shape pathways and contribute to service innovation.

Essential Requirements

• Degree or recognised qualification in Occupational Therapy and registration with HCPC.
• Relevant post‑qualification experience in an Occupational Therapy role.
• Experience of occupational therapy assessment and intervention in health, social care, or community settings.
• Experience working with children, young people, or adults with learning disabilities, autism, physical disabilities, mental health needs, or complex needs.
• Knowledge of relevant legislation.
