Job summary

You will work in a hybrid pattern, with one day in the office, and collaborate with a multidisciplinary team to support community care and ensure positive outcomes for people in need.

Responsibilities

• Conduct comprehensive Occupational Therapy assessments in service users' homes.
• Set and work towards meaningful goals to maximise independence.
• Deliver specialised Occupational Therapy interventions tailored to individual needs.
• Monitor progress and adapt intervention strategies to achieve positive outcomes.
• Aid service users in regaining confidence and improving quality of life.
• Perform reablement-focused assessments and reviews for short-term support.
• Apply evidence-based Occupational Therapy practices effectively.
• Coordinate with reablement care providers for seamless care transitions.
• Liaise with various health and social care professionals ensuring a collaborative approach.
• Manage a diverse community caseload with professionalism.
• Document all assessments, reports, and case details accurately.
• Engage in multidisciplinary teamwork and collaborative care planning.
