Job summary

Qualifications

• Essential: Relevant post qualifying experience.
• Registration with HCPC with a recognised Occupational Therapy qualification.
• Completion of safeguarding training Level 2/3.

Responsibilities

• Carry out holistic assessments, setting meaningful reablement goals and developing personalised support plans.
• Work closely with individuals, carers and partner organisations to support timely hospital discharge.
• Promote independence and wellbeing through person-centred, strengths-based practice.
• Manage a varied caseload with sound decision-making and composure under pressure.
