• Assess, plan, deliver and evaluate rehabilitation and therapy interventions for adults within acute, community and home settings.
• Manage a varied clinical caseload, undertaking holistic assessments and treatment planning.
• Support safe and timely discharge planning and admission avoidance, reducing readmissions.
• Contribute to workforce development and supervise therapy assistants and students as appropriate.
