Job summary

Bracknell Forest Council is seeking a HCPC-registered Occupational Therapist to join the Early Intervention and Prevention Team on a 12-month fixed-term contract. You will carry a varied caseload, conduct holistic assessments and plan reablement to maximise independence, with flexible and hybrid working options.

The role requires HCPC registration, manual handling experience, and providing person-centred, strengths-based support to residents to optimise outcomes and safe discharges.

Qualifications

• HCPC registered Occupational Therapist.
• Experience with manual handling.
• Ability to manage a varied caseload and make sound decisions.
• Commitment to person-centred, strengths-based practice.

Responsibilities

• Manage your own caseload, completing timely, person-centred assessments and reablement plans.
• Set SMART goals with individuals and carers to maximise independence.
• Act as autonomous practitioner with accurate records and safeguarding where required.
• Support timely hospital discharge and prevent avoidable admissions.
