Bracknell Forest Council is seeking an HCPC registered Occupational Therapist to join the Early Intervention and Prevention Team on a 12-month fixed-term contract. You will assess, plan and support reablement to maximise independence, with a focus on safe hospital discharge and community perseverance.

The role offers flexible hours, hybrid working with 2–3 days in Times Square office, and a supportive environment with opportunities for professional development.

Qualifications

• HCPC registration required.
• Experience in occupational therapy including manual handling.
• Able to manage a varied caseload and work under pressure.

Responsabilités

• Manage own caseload with holistic assessments and reablement plans.
• Collaborate with individuals, carers and colleagues to set SMART goals to maximise independence.
• Support timely hospital discharge and prevent avoidable admissions.
• Maintain accurate records and contribute to safeguarding assessments.
