Job summary

Cambridgeshire and Peterborough NHS Foundation Trust seeks a Nursing Associate to deliver frailty and patient‑centred care. You’ll undertake delegated assessments and monitor patients’ physical, psychological and social needs, escalating concerns promptly.

You’ll collaborate with the Frailty Lead, Pharmacy and Personalised Care teams, perform home visits, support discharge reviews, continence, nutrition and mobility, and help prevent unnecessary hospital admissions through early intervention.

Responsibilities

• Assess frailty and holistic patient needs and escalate changes promptly.
• Monitor vital signs, wound care and continence within scope.
• Support medicines-related care and home visits for high-risk patients.
• Coordinate with Frailty Lead, Pharmacy and Personalised Care teams.
• Promote health, prevention and appropriate community interventions.
• Maintain accurate clinical records and document interactions.
