Job summary

Clayton Brook Surgery is seeking an enthusiastic Advanced Nurse Practitioner to join our General Practice team on a part-time basis (16–24 hours per week). The role involves autonomous clinical assessment, managing a broad range of conditions, and acting as the main clinical contact for one of our care homes, including a weekly ward round in collaboration with GPs and the wider MDT.

Qualifications

• Registered Nurse with Advanced Nurse Practitioner/Advanced Clinical Practice qualification.
• Experience in General Practice or Primary Care.
• Experience of autonomous clinical assessment and decision-making.
• Ability to assess and manage patients presenting with acute and ongoing health conditions.
• Excellent communication and interpersonal skills.
• Ability to work autonomously and as part of a multidisciplinary team.
• Ability to work within professional competence and scope of practice.
• Ability to establish an effective working relationship with the allocated care home and undertake the weekly ward round.

Responsibilities

• Undertake autonomous clinical assessment of patients presenting with acute and ongoing health problems.
• Take comprehensive patient histories and conduct appropriate examinations.
• Formulate differential diagnoses and develop treatment plans.
• Prescribe medications within competence and scope of practice.
• Provide advice, health promotion and preventive care.
• Support care home residents with proactive monitoring and escalation as needed.
• Collaborate with GPs, nurses and the MDT to ensure coordinated care.
• Maintain accurate clinical records.
