Job summary

Drs Reddy & Nunn is recruiting an Advanced Nurse Practitioner to join our established team serving Bridlington and surrounding areas. You will provide autonomous advanced clinical assessment, diagnosis and care planning for care home residents and housebound patients with complex needs.

The role offers flexible hours (full-time preferred, part-time considered) within a forward-looking practice committed to person-centred care, frailty management and integrated community services.

Qualifications

• Experience working within care homes.
• Experience in primary care, community services, urgent care or frailty services.
• Experience of Enhanced Health in Care Homes (EHCH).
• Experience in service development or quality improvement projects.
• Knowledge of frailty, multimorbidity and complex care management.

Responsibilities

• Provide advanced clinical assessment, diagnosis and care planning for care home residents and housebound patients.
• Support care homes and the practice team through clinics and service delivery as required.
• Act as a clinical link between primary care, care homes, community services and partner organisations.
• Manage frailty, long-term conditions and urgent healthcare needs.
