Job summary

East Of England Community Health And Care Nhs Trust is recruiting a Senior Community Nurse to deliver expert home-based care for patients with complex health needs in King's Lynn. You will manage long-term conditions, frailty and end-of-life care, promoting independence at home.

Working autonomously within an MDT, you will lead clinical care planning, supervise newly qualified nurses through structured preceptorship, and help avoid hospital admissions through timely discharge and integrated care.

Qualifications

• Assessor/mentorship preparation ENB 998 or equivalent.
• Current NMC registration.
• Independent or supplementary prescribing qualification.
• Evidence of ongoing CPD.
• Experience at band 5 level in the community setting.

Job summary

An exciting opportunity has arisen for a Senior Community Nurse to join our friendly and supportive Community Nursing Team. In this role, you will provide expert nursing care to patients in their own homes, supporting individuals with complex health needs, long-term conditions, frailty and end-of-life care.

Working autonomously and as part of a multidisciplinary team, you will undertake comprehensive assessments, develop and evaluate person-centred care plans, and coordinate care to help patients remain safely at home. You will play a key role in admission avoidance, helping to facilitate timely hospital discharge, and promoting independence and self-management.

As a senior member of the team, you will provide clinical leadership, support service development, and contribute to the supervision and development of newly qualified nurses through structured preceptorship and mentorship.

Main duties of the job

To contribute in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community and in clinics, by assessing, planning, implementing and evaluating packages of care. To facilitate/support newly Qualified Nurses in their Preceptorship year.

Job responsibilities

To assess, prescribe and deliver care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.

To work within the integrated team to facilitate early discharge from hospital.

To work within the integrated team to prevent unnecessary admission to hospital.

To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.

Qualifications
