An exciting opportunity has arisen for an Advanced Clinical Practitioner to join our Mearns cluster. We are seeking an exceptional individual who can join us in our continued journey to deliver excellence in care throughput Aberdeenshire.
The role of the Advanced Clinical Practitioner is a diverse and autonomous clinical role. This role will deliver positive outcomes for patients by providing enhanced clinical assessment and treatment which will meet the needs of patients within the community setting. You will support the medical staff and other healthcare professionals in the community to provide optimum service delivery. ACPs work in collaboration with independent practices to provide high-level care throughout clusters, undertaking acute on-the-day triage and home visits when required.
This post offers an exciting opportunity for an experienced registered professional to work across multiple clinical areas, including GP surgeries, community hospitals, and patients' homes.
We seek a suitably qualified Advanced Nurse Practitioner/Advanced Clinical Practitioner with evidence of advancing practice, a sound understanding of conditions requiring hospital admission or virtual community ward management, and the ability to recognize and initiate management of deteriorating patients. Experience managing patients at home as part of an MDT team is also essential. Applicants should be proactive in expediting patient journeys and developing the role of the ACP within Aberdeenshire.
Educational requirements align with The Advanced Care Academy framework, including a portfolio, which must be completed within an agreed timeframe. Consideration may be given to training roles for candidates demonstrating a commitment to advanced practice and academic progression.
Hours are flexible to meet service needs; part-time hours will be considered.
A valid driving license and access to a vehicle are essential, as travel within the Mearns cluster is required.
