An exciting opportunity has arisen for an Advanced Clinical Practitioner to join our Mearns cluster. We seek an exceptional individual committed to delivering excellence in care within Aberdeenshire.
The role of the Advanced Clinical Practitioner is diverse and autonomous, focusing on positive patient outcomes through enhanced clinical assessment and treatment in the community setting. You will support medical staff and other healthcare professionals, collaborating with independent practices to provide high-quality care across clusters, including acute triage and home visits.
This position offers an excellent opportunity for an experienced registered professional to work across various clinical environments, such as GP surgeries, community hospitals, and patients' homes.
We require a suitably qualified Advanced Nurse Practitioner/Advanced Clinical Practitioner with evidence of advancing practice and a solid understanding of conditions that may necessitate hospital admission or management within the virtual community ward. Experience in managing patients at home as part of an MDT team is also essential. Applicants should be proactive in streamlining patient journeys and developing the role of the ACP in Aberdeenshire.
Educational requirements align with The Advanced Care Academy framework, including a portfolio, which must be completed within an agreed timeframe. Consideration may be given to training roles for those committed to advancing their practice and beginning their academic journey.
Flexible working hours are required to meet service needs, with part-time hours considered. A valid driving license and access to a vehicle are essential, along with willingness to travel within the Mearns cluster.
