An exciting opportunity has arisen for an Advanced Clinical Practitioner to join our Mearns cluster. We are seeking an exceptional individual who can join us in our continued journey to deliver excellence in care throughput Aberdeenshire.

The role of the Advanced Clinical Practitioner is a diverse and autonomous clinical role. This role will deliver positive outcomes for patients by providing enhanced clinical assessment and treatment which will meet the needs of patients within the community setting. You will support the medical staff and other health care professionals in the community to provide optimum service delivery. ACP’s work in collaboration with independent practice’s to provide a high level of care throughout clusters. Undertaking acute on the day triage and home visits when required.

This post will provide an exciting opportunity for an experienced registered professional to work within multiple clinical areas, including, GP surgeries, community hospitals and the patients own home.

We are looking for a suitably qualified Advanced Nurse Practitioner/ Advanced Clinical Practitioner to join our team where upon evidence of advancing practice is essential for the role, with a sound understanding of the range of conditions that may require patients to be admitted to hospital or the virtual community ward and recognise and initiate clinical management of the deteriorating patient. Additionally, experience and clinical knowledge base for managing patients in their own home (as part of an MDT team). Applicants should be proactive in expediting the patient journey and developing the role of the Advanced Clinical Practitioner within Aberdeenshire as part of a multidisciplinary team.

Educational requirements are in adherence with The Advanced Care Academy framework which includes a portfolio. The completion of this will be mandatory within an agreed time frame. Considerations may be given to a training role for those who are able to demonstrate a commitment to advanced practice and have begun their academic journey.

Hours of work are required to be flexible to meet service needs. Part time hours will be considered.

A valid driving licence and access to a vehicle is essential. Willingness to travel within the Mearns cluster are essential.
